By Provider Partners Connect Care News
Long-term care is in the midst of a meaningful shift. Residents are entering care with increasingly complex needs. Staffing continues to stretch thin, and the demand for improved outcomes and sustainable solutions continue to grow. These realities require a new generation of leadership that is adaptive, grounded, and ready to act.
We are proud to announce Eric Palm, Chief Growth Officer at Provider Partners Connect Care, as a member of the WTWH Healthcare Future Leaders Class of 2025. Eric exemplifies the next wave of long-term care leadership: professionals under 40 who combine innovation with empathy, leverage technology without losing human connection, and guide providers and clinicians with clarity and
confidence.
A Model That Works and a Leader Who Knows Why
Eric leads the growth of our High-Needs ACO REACH program, a purpose-built value-based care solution tailored to Medicare beneficiaries with complex needs. Unlike traditional programs retrofitted from acute care models, this program was designed from the ground up for long-term care facilities and the clinicians who care for their residents.
Through his leadership, Eric helps long-term care teams understand how the program fits into their daily operations and why our approach makes transitioning to value-based care more practical and sustainable. He clearly demonstrates how the program supports high-acuity residents, integrates with existing workflows and offers the structure needed for real progress. His focus stays on the priorities that matter: flexibility in clinical practice, operational efficiency, and long-term financial alignment.
Supported by a team of seasoned value-based care professionals, Eric communicates the program’s strengths with clarity. Features such as remote RN care coordination and a no-risk participation model position the program as a trusted, results-driven solution for LTC teams ready to embrace meaningful change.
Grounded in Experience, Driven by Perspective
Eric attributes much of his success to the mentorship of our founder, a former nursing home medical director with decades of experience inside long-term care. That relationship offered Eric a direct view into the challenges, pressures, and responsibilities faced by today’s clinicians and operators. It shaped how he sees the work; not as a set of metrics or models, but as a system of people who need support, understanding, and solutions that truly fit.
This foundation, combined with Eric’s natural ability to connect, has made him a leader who blends technical expertise with empathy. He brings a thoughtful, tech-savvy mindset to the table while remaining approachable, grounded, and focused on building trust across every conversation.
A Team That Makes VBC Work
Eric, along with his fellow senior leaders, is supported by a dedicated team of operational and clinical professionals who share a clear mission: to make value-based care work for long-term care. This team has built a program that simplifies the transition, respects the clinical judgment of care teams, and scales across facilities with varying needs.
What Eric adds to this foundation is momentum. He brings energy and clarity to every conversation, keeping initiatives moving forward without adding complexity. His role as Chief Growth Officer is not simply about metrics or performance. It is about helping remove friction so that long-term care leaders can move confidently toward meaningful change.
Leading with Heart and Focus
WTWH’s Future Leaders program honors professionals under 40 who are shaping the future of post- acute care. Eric reflects that mission completely. He is focused without ego, approachable without effort, and committed to the people doing the work.
As Eric puts it, “The goal is not to add more to anyone’s plate. It is to offer a solution that fits the way long-term care teams already operate and make it easier to succeed.”
We are proud to see Eric recognized among a class of changemakers working across healthcare to create what comes next. His leadership reflects the best of what value-based care can be: connected, practical, and deeply human.
