PPCC Sees Opportunity with CMS’s New LEAD Model Launching in 2027

Recently, the Centers for Medicare & Medicaid Services Innovation Center (CMMI) announced LEAD Model (Long-Term Enhanced ACO Design), scheduled to launch in 2027. As CMMI continues to advance value-based care, the new model represents a meaningful next step for organizations serving Medicare beneficiaries, particularly in nursing homes and long-term care settings.

Keith Persinger, CEO of Provider Partners Connect Care (PPCC), shared his perspective with Park Place Live, a leading industry publication, on what the LEAD Model may mean for providers, facilities and the broader long-term care ecosystem.

As the specifics of the new LEAD model are announced during 2026, we believe this model will further support the goals of the existing High Needs REACH model while providing new enhancements to healthcare providers and practices to improve patient care and outcomes.

The LEAD Model is part of CMS’s ongoing commitment to creating innovative payment structures that enhance care coordination for nursing home residents, reduce avoidable hospitalizations, and improve quality of care through stronger integration between nursing facilities and primary care providers.

For ACOs, nursing facilities and provider networks serving long-term care populations, the LEAD Model introduces new opportunities for strategic planning and operational readiness as healthcare continues its shift toward value-based care.
